- Fees
- Fees can be found on the booking page.
- Direct Billing Insurance
- We are not set up for direct billing. Receipts are issued after the visit with all the information necessary for patients to submit to their insurance provider.
- Credit Card
- We require a credit card to be held on file for your appointments. If you are not comfortable leaving a credit card number on file, we cannot book you an appointment.
- Cancellation Policy
- We have a 2 day cancellation and rescheduling policy at our office. Appointments that are cancelled or rescheduled within 48 hours of the visit, will be charged 50% of the visit fee. Those that are missed, cancelled, or rescheduled on the same day as the visit are charged the full rate for the appointment.
